Please Read If You Are Attending Mass In Person
You will be asked to sign an acknowledgement of these guidelines before entering Mass. Attached for your convenience is a copy of the guidelines for printing.
1. In the past 14 days, have you experienced any of the following new, unusual, or worsening symptoms?
- Fever, Chills (temperature ³ 37.8°C/100°F)
- Cough
- Trouble breathing
- Sinus congestion or Runny nose
- Sore throat
- Nausea & Vomiting
- Diarrhea
- Unexplained muscles aches
- Loss of taste or smell
- Eye redness or discharge
(unusual = not explained by a pre-existing condition)
2. In the past 30 days, have you been diagnosed with COVID-19?(diagnosed = diagnosis or confirmed by physician/provider)3. In the past 14 days, have you had unprotected close contact with someone diagnosed with COVID-19 (such as a household contact)?(unprotected = without a face covering or without appropriate PPE at work if a health care worker?)(close contact = within 6 feet for 15 minutes or longer, live with a person diagnosed with COVID-19)

- Stay home if you have a fever or symptoms not explained by other pre-existing conditions
- Fill out health questionnaire at home and bring it with you to the sign-in table
- Your temperature will be taken before you enter the sanctuary
- Enter Mass using the main doors
- Maintain social distancing; use hand sanitizer; keep MASK ON at all times
- Fill out form at sign-in table ONLY if you did not bring your questionnaire from home
- Drop your donation in the collection basket as you exit church
- Go to seating in 6-feet-apart chairs (unless you are with members of your household)
- Singing will be only by cantor; you may hum and/or clap
- Communion will be distributed at the end of Mass; keep mask on until presider moves away from you, then consume the host
- Exit Mass using the side door
Note: In accordance with CDC and local guidelines, everyone Must Wear Mask in order to distribute and receive Communion. Those receiving Communion should receive communion then step away from the line before removing their mask to consume Holy Communion.Please help us facilitate the check in process by completing the Screening Questions below AND BRING IT WITH YOU TO MASS!

1. St. Columba Church will phase in small groups for opening the church for Mass.
2. 100 People will be inside the Church on Sunday at 10:30am.
3. Everyone will have to reserve on Eventbrite to attend Mass and must bring their ticket or show proof of registration on their phones.
You will be asked to sign an acknowledgement of the attached COVID-19 screening questions before entering Mass.
You may print out and bring a signed copy of the questionnaire with you to Mass.

CCHS expects to open eligibility to those 16 years or older later this week. Timing for that decision will depend on how quickly available appointments fill in the coming days.
People who are currently eligible are strongly encouraged to beat the rush by requesting appointments as soon as possible, using the online request form at cchealth.org/coronavirus or calling 1-833-VAX-COCO (1-833-829-2626).
People currently eligible for appointments in Contra Costa include:
- Anyone who is 50 or older and lives or works in the county
- Anyone who is 16 or older and has a high-risk health condition, disability or illness and lives or works in the county
- Essential workers who live or work in Contra Costa, including healthcare personnel, emergency services workers, education and childcare workers, agriculture and food workers, transportation and logistics workers, and anyone who lives or works in a congregate living space in the county
CCHS expects this week to expand eligibility to:
- Anyone who lives or works in the county and is 16 or older
Appointments are still required at CCHS vaccine clinics, and each patient must have their own appointment.
CCHS does anticipate a sharp rise in requests when additional eligibility is added, and long delays are likely on the appointment request phone line. Anyone who is able to do so is encouraged to use the online request form instead of phoning – it will save you time.
People who live in Contra Costa communities that are disproportionately impacted by the COVID-19 pandemic, including communities of color, are especially encouraged to request an appointment today.

Here are additional updates on vaccine distribution in our counties and by healthcare providers:
Contra Costa County Contra Costa County residents 65 and older, along with healthcare workers who have not yet been vaccinated, can request vaccine appointment by filling out this online form. You don't need to have health insurance. After submitting the form, you will be notified with an appointment date and time for one of the county-run sites, including one in Richmond.
Alameda County Alameda County is now scheduling appointments at two county-run sites for county residents 65 and older and essential workers in education and childcare, emergency services, and agriculture and food industries, along with healthcare workers who have not yet been vaccinated. Sign up to get an appointment on the online registry system. And, no need for insurance to get a vaccine.
Berkeley City of Berkeley's Health Department is operating a mass vaccination site at the foot of Buchanan Street in Albany near Golden Gate Fields. Appointments are for people 65 and older and those who live or work in Berkeley who are currently employed in grocery stores, convenience stores, or in-person education and childcare settings. - Vaccinating Kaiser members 75 and older, health care workers, and long-term care patients and staff
- Sign in online to check vaccine status or schedule an appointment
Sutter
- Sutter patients 65 and older can go online, log in to My Health Online, or call 844-987-6115 to schedule a vaccine appointment
John Muir Health
- John Muir Health will contact its patients via MyChart message, email, text message or phone call to schedule an appointment, currently vaccinating age 65 and older. Patients encouraged to sign up for a MyChart account by visiting the MyChart sign up information page.
Community Clinics
- Native American Health Center is vaccinating patients and community members 65 years old and older.
- Lifelong is reaching out to patients who are 1) over age 65 or 2) IHSS (In-Home Support Services) workers or 3) food and agricultural workers with instructions for making an appointment. If you are a registered patient of LifeLong and have not been contacted, please contact your health center.
- Asian Health Services is providing vaccine appointments to AHS patients 65 years and older or are IHSS workers. Patients can sign up for vaccine wait list here.
- La Clínica is vaccinating patients who are IHSS workers, caregivers of people with disabilities, or 65 and older. Eligible patients are being contacted by phone or can call their La Clínica provider to request an appointment.
- West Oakland Health Center is vaccinating patients 65 and older.
CVS and Rite Aid Select CVS and Rite Aid pharmacy locations are also accepting appointments for vaccines. For CVS, go here, and for Rite Aid, click here.
Mobile Clinics FEMA is also dispatching mobile vaccine clinics throughout the East Bay, particularly to communities that have been hit hardest by COVID and are under-served by our healthcare system. I'll send out more information about the mobile clinics when it becomes available.
Please continue to be safe - wear a mask and social distance - even after you've been vaccinated, because we don't know yet whether people who have been vaccinated can still spread the disease.
