Attached please find the annual Self-Assessment Form used by parishes and schools as part of our annual Compliance Audit with the USCCB. The annual audit required that we submit completed forms to the USCCB during the summer so we need your information no later than the June 15 deadline. In order to avoid confusion, please ONLY send them to Tina Romeo by fax or US mail (see details at the bottom of every page.)

If you are a new pastor or administrator or principal, please contact Ray Delisle with any questions you may have in order to complete the forms in time.
Also, please note that the Office of Religious Education already included the annual forms for Religious Education in a recent issue of Update+ to Parish Catechetical Leaders.
