my name is Darren Cools. I am the creative director at the Archdiocese of Portland, and I can answer your questions.
Regarding image use:
1. A content manager (for print or web) wishing to use an image of any person on any form of media accessible to the public with the intention of promoting their own entities or events must receive a written release form (signed by a parent or guardian if the person is under 18), granting permission to use their image for self promotional purposes.
2. If a content manager wishes to create a space where participants of an event or organization can access images of friends and activities, it is permissible to use images without a signed release form, but ONLY for non-commercial and non-promotional purposes—i.e., for community purposes only.
3. It is always preferable, regardless of use, to request release forms.
4. In ALL cases, regardless of use and intent, images minors and children should be accompanied by a release form.
This is official stance taken by most professionals and by the Archdiocese of Portland. I have attached the release form we use internally. You can also find template forms easily on the internet which can be modified for your own use.
