The Archdiocesan Office of Catholic Schools is gathering data to assess the climate of our school. As a parent/guardian, you are invited to provide data concerning the following items based on your personal knowledge/experience. Your name must be entered to submit the form. This is to insure that the professional evaluator can contact you in the event they have questions regarding your rankings or comments. Unsigned forms will not be included in the final assessment. Thanks for your input. All surveys must be completed by April 8.
